Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has brought about the coronavirus disease 2019 (COVID-19) that adversely impacted global health. The emergence of SARS-CoV-2 first took place in Wuhan, China, in late 2019. It was declared a global pandemic by the World Health Organization (WHO) in early 2020. Most people infected by the virus experience either asymptomatic or mild to moderate respiratory disease. However, critical cases of COVID-19 have been observed in older people and people with comorbidities that cause pneumonia and ultimately death of the patients. Recently, COVID-19 has also been responsible for causing gastrointestinal diseases and neurological complications.<\/p>\n\n\n\n

The detection of COVID-19 takes place by a collection of swab samples from the nose or throat and carrying out a Reverse Transcriptase-Polymerase Chain Reaction (RT-PCR). This test helped detect viral RNA in the swab sample that indicated that the virus has infected the individual. Certain rapid antibody detection kits are also available that could detect positive cases. However, RT-PCR results are considered to be most important in the detection of COVID-19.<\/p>\n\n\n\n

Origin of COVID-19<\/h2>\n\n\n\n

Coronaviruses are single-stranded, positive-sense enveloped RNA viruses that belong to the Coronaviridae family. They are capable of causing infections in both animals and humans. Coronaviruses are known to belong to four different genera: Alpha, Beta, Gamma, and Delta Coronavirus. The alpha and beta forms infect rodents and bats, while the gamma and delta forms infect birds.<\/p>\n\n\n\n

Three different coronaviruses have caused major outbreaks of infection as well as fetal pneumonia throughout the world since the beginning of the 21st century. The first outbreak was caused by Severe Acute Respiratory Syndrome Coronavirus (SARS-CoV-1) in 2002 at Foshan, China. The second outbreak was caused by the Middle East Respiratory Syndrome Coronavirus (MERS-CoV) in 2012 in Jeddah, Saudi Arabia. The third outbreak was caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) in 2019 at Wuhan, China. All three viruses have a zoonotic origin; SARS-CoV-2 especially was first detected in bats.<\/p>\n\n\n\n

SARS-CoV-2 is the seventh coronavirus known to infect humans. The pathogenicity of SARS-CoV-1, MERS-CoV, and SARS-CoV-2 is higher than the other coronaviruses since they affect the respiratory tract leading to severe pneumonia and extra-pulmonary diseases.<\/p>\n\n\n\n

Genome Organization of SARS-CoV-2<\/h2>\n\n\n\n

The genome of SARS-CoV-2 comprises 26-32 kilobase (kb) non-segmented positive-sense single-stranded RNA. It consists of 13 to 15 open reading frames (ORFs), out of which 12 are functional. Two untranslated regions (UTRs), 5\u2032 UTR and 3\u2032 UTR are present that help bind viral and host proteins. SARS-CoV-2 genome is also capable of binding with a few host microRNAs.<\/p>\n\n\n\n

ORF1a codes for 12 non-structural proteins, while the downstream of ORF1b codes for the major structural proteins. The major structural proteins are: spike (S) glycoprotein, envelope (E), membrane (M), and nucleoprotein (N) proteins.<\/p>\n\n\n\n

The S protein is most important in viral pathogenesis and infection since it promotes entry of the virus inside the host cell. M is the most abundant glycoprotein that helps in the transport of nutrients. The N protein stabilizes and protects the viral genome, and the E protein helps in the release of the virus.<\/p>\n\n\n\n

COVID-19 First Wave<\/h2>\n\n\n\n

Since its emergence, SARS-CoV-2 has brought about two waves of the pandemic. The onset of the first wave of the pandemic was observed during the last week of March 2020 in India. However, the first cases of COVID-19 in India were reported in January 2020 in three towns of Kerala. Three medical students, who had returned from Wuhan, were responsible for the outbreak of COVID-19 in Kerala. The government imposed a nationwide lockdown during the latter part of March 2020, leading to the closure of schools, colleges, offices, and all other public places. Only those shops selling essential commodities were allowed to be kept open. Gathering of any sort was prohibited during the lockdown.<\/p>\n\n\n\n

India faced a peak of cases during April and May 2020. In June 2020, the number of recoveries started exceeding the number of active cases. The number of cases began to subside significantly during September 2020. During the initial months of the pandemic, the only means of controlling transmission of the virus were wearing masks, personal protective equipment (PPE), and maintaining social distance. Critical patients were initially treated with hydroxychloroquine and required oxygen supply and ventilation. The antiviral drug approved by FDA for emergency use was remdesivir. Clinical plasma therapy was also used to treat some patients initially when limited treatment options were available.<\/p>\n\n\n\n

Although certain treatment options existed, they were not highly effective. Scientists around the world considered the development of vaccines as the only way of curbing the pandemic. The development of vaccines began in India in November 2020. In January 2021, two vaccines received approval for use: Covishield developed by Serum Institute of India and Covaxin developed by Bharat Biotech.<\/p>\n\n\n\n

COVID-19 Second Wave<\/h2>\n\n\n\n

The second wave of the pandemic was much more destructive than the first wave. The second wave was caused due to a mutated version of SARS-CoV-2 \u2013 the Delta strain. This strain had much higher infectivity and virulence as compared to the original strain of SARS-CoV-2. The second wave hit India in March 2021. However, nationwide lockdown did not occur during the second wave, instead state-wise lockdown and restrictions were imposed.<\/p>\n\n\n\n

Although mass vaccination had begun by that time, only the healthcare workers and old-age adults were eligible to get vaccinated. The devastation caused by the second wave was much more compared to the first wave since most of the population was susceptible to it. The second wave observed the crumbling of the healthcare systems in India, with a shortage of oxygen supply, hospital beds, vaccines, and other medical facilities in almost all parts of the country.<\/p>\n\n\n\n

The number of deaths reported due to the second wave was also much higher than the first wave. However, the active cases began to subside in May 2021, following which the government implemented the vaccination program for all individuals above the age of 18 years. In November 2021, schools, colleges and offices had started opening in many parts of the country, as the number of cases decreased significantly. Experts believe that India has now begun to enter the endemic stage of the pandemic.<\/p>\n\n\n\n

COVID-19 Third Wave<\/h2>\n\n\n\n

Although most of the population is currently vaccinated, there is still a possibility of an incoming third wave, as mentioned by a few doctors across the country. Most people across India have stopped following COVID-19 protocols as they believe complete vaccination will ensure no further infection. However, vaccinated individuals can contract COVID-19 infection, although the infection might not be that severe.<\/p>\n\n\n\n

Few experts believe that even if a third wave hits India, it will be much weaker than the first and second waves. In addition, the Indian government has stated that they are better prepared to tackle the third wave if and when it occurs. Although few experts had stated previously that the third wave is most likely to hit during September-October 2021, there have been no signs of it yet. Therefore, few people believe that there has been unwanted panic and paranoia regarding it without any evidence. Instead, people must now focus on following COVID-19 restrictions.<\/p>\n\n\n\n

An increase in the number of COVID-19 cases has been observed following certain festivities in some of the states in India. Furthermore, the relaxation of restrictions has led to overcrowding in public places. Experts believe that people must be cautious since the pandemic is not over yet. Even though vaccinated, they must follow COVID-19 protocols to ensure no further waves hit India and cause more devastation.<\/p>\n\n\n\n
